as a U.S. Marshal named Edward "Teddy" Daniels, who is sent to a high-security psychiatric hospital called Ashecliffe to help track down a missing patient named Rachel Solando.
Teddy and his new partner, Chuck Aule (

), are shown around the island by its director, Dr. John Cawley (
Ben Kingsley
). But the pair quickly realize that Dr. Cawley and his staff are largely uncooperative as they attempt to learn more information that will help them trace Solando’s whereabouts.
Teddy reveals to Chuck that he is suspicious of the hospital’s leaders, whom he believes are mistreating the patients and hiding a man named Andrew Laeddis. As Teddy explains, Laeddis is an arsonist who started a fire that ultimately killed his wife, Dolores Chanal (

), and he's the real reason he took the case on the island.

Shutter Island
, it is revealed that Teddy is actually a longtime patient of Ashecliff’s named Andrew Laeddis, who created the persona of Teddy Daniels to escape reality.
Dr. Cawley explains that while Andrew was once a U.S. Marshal, he was relieved of his duties two years prior after he shot his wife Dolores for drowning their three children in the lake behind their home.
Dr. Cawley and Dr. Lester Sheehan, who Andrew knows as his “partner” Chuck, tell Andrew that they have been engaged in an elaborate role-playing scenario with him, involving the entire hospital staff and allowing him free reign of the grounds.
The doctors explain their hope that by allowing Andrew to act out his Teddy fantasy, he might finally come to grips with his true identity, preventing an otherwise necessary lobotomy to subdue his violent tendencies.
Though Andrew is initially disbelieving, he is able to remember the crime he committed after Dr. Sheehan urges him to acknowledge the existence of his daughter, Rachel.
The following morning, however, the doctors’ hopes are dashed when Andrew greets Dr. Sheehan as “Chuck” and begins to reiterate his belief that they must escape from the island together.
Sheehan exchanges a glance with Cawley, who presumably orders Andrew to be taken away for a lobotomy.

Tragically, they are killed by their mother, who drowns them in a manic depressive episode while their father is away on business.
Though Andrew eventually returns home, he is too late to save his kids. In a moment of disbelief and anger, he reacts to their deaths by shooting and killing his wife.
In addition to his guilt over murdering Dolores, Andrew blames himself for the death of their children, as he tells doctors that he ignored the warning signs of his wife’s mental decline, which included burning down their home.

, Andrew makes a statement that prompts the audience to question whether or not he’s actually sane and pretending to be otherwise.
Though he greets his doctor by the name of “Chuck” — the fictional partner he’s created for his alter ego — he follows it up with a thought-provoking question about their surroundings that he’s been pondering: “Which would be worse? To live as a monster or to die as a good man?"
While the line ultimately leaves viewers to decide whether Andrew is lucid and choosing to “die as a good man” by being lobotomized instead of living as the monster he believes he is, Dennis Lehane, the author of the book the film is based in, has weighed in with his thoughts on what the line means.
According to Lehane, who ended the book with Andrew's brief lucidity quickly passing, the film’s line is merely a “momentary flash” of the character’s sanity.

"My feeling was no, he’s not so conscious he says, ‘Oh I’m going to decide to pretend to be Laeddis so they’ll finally give me a lobotomy.’ That would just be far more suicidal than I think this character is,” he told MTV News (via
IndieWire
).
Lehane continued, “I think that in one moment, for a half a second sitting there in that island he remembered who he was and then he asks that question and he quickly sort of lets it go. That was my feeling on that line.”
According to the film’s psychiatric adviser, Professor James Gilligan, however, who spoke to
The Guardian
in 2010, Andrew was aware of his actions when he was ordered to be lobotomized.
The doctor revealed that he interpreted the line to mean, “I feel too guilty to go on living. I'm not going to actually commit suicide, but I'm going to vicariously commit suicide by handing myself over to these people who're going to lobotomize me."

More than a decade after the release of the 2010 psychological thriller *Shutter Island*, its ending remains a mystery to many.
The [Martin Scorsese](https://people.com/tag/martin-scorsese/)\-directed film stars [Leonardo DiCaprio](https://people.com/tag/leonardo-dicaprio/) as a U.S. Marshal named Edward "Teddy" Daniels, who is sent to a high-security psychiatric hospital called Ashecliffe to help track down a missing patient named Rachel Solando.
Teddy and his new partner, Chuck Aule ([Mark Ruffalo](https://people.com/tag/mark-ruffalo/)), are shown around the island by its director, Dr. John Cawley ([Ben Kingsley](https://people.com/tag/ben-kingsley/)). But the pair quickly realize that Dr. Cawley and his staff are largely uncooperative as they attempt to learn more information that will help them trace Solando’s whereabouts.
Teddy reveals to Chuck that he is suspicious of the hospital’s leaders, whom he believes are mistreating the patients and hiding a man named Andrew Laeddis. As Teddy explains, Laeddis is an arsonist who started a fire that ultimately killed his wife, Dolores Chanal ([Michelle Williams](https://people.com/tag/michelle-williams/)), and he's the real reason he took the case on the island.
As his frustrations grow, however, so do his chances of leaving the island of his own free will without interference from the nefarious staff.
So what really happened at the end of *Shutter Island*? In honor of the movie celebrating its 15th release anniversary on Feb. 19, here's everything to know about the psychological thriller and what that chilling final line signified about the main character’s sanity.
**Warning: *Shutter Island* spoilers ahead\!**
## How did *Shutter Island* end?

At the end of *Shutter Island*, it is revealed that Teddy is actually a longtime patient of Ashecliff’s named Andrew Laeddis, who created the persona of Teddy Daniels to escape reality.
Dr. Cawley explains that while Andrew was once a U.S. Marshal, he was relieved of his duties two years prior after he shot his wife Dolores for drowning their three children in the lake behind their home.
Dr. Cawley and Dr. Lester Sheehan, who Andrew knows as his “partner” Chuck, tell Andrew that they have been engaged in an elaborate role-playing scenario with him, involving the entire hospital staff and allowing him free reign of the grounds.
The doctors explain their hope that by allowing Andrew to act out his Teddy fantasy, he might finally come to grips with his true identity, preventing an otherwise necessary lobotomy to subdue his violent tendencies.
Though Andrew is initially disbelieving, he is able to remember the crime he committed after Dr. Sheehan urges him to acknowledge the existence of his daughter, Rachel.
The following morning, however, the doctors’ hopes are dashed when Andrew greets Dr. Sheehan as “Chuck” and begins to reiterate his belief that they must escape from the island together.
Sheehan exchanges a glance with Cawley, who presumably orders Andrew to be taken away for a lobotomy.
## What happened to the kids in *Shutter Island*?

Andrew and Dolores welcomed three children together, including two sons and a daughter named Rachel.
Tragically, they are killed by their mother, who drowns them in a manic depressive episode while their father is away on business.
Though Andrew eventually returns home, he is too late to save his kids. In a moment of disbelief and anger, he reacts to their deaths by shooting and killing his wife.
In addition to his guilt over murdering Dolores, Andrew blames himself for the death of their children, as he tells doctors that he ignored the warning signs of his wife’s mental decline, which included burning down their home.
Unable to face the horrors of his reality, Andrew creates an alter ego named Teddy to escape his upsetting memories.
## Was Andrew sane at the end of *Shutter Island*?

In the [final moments of *Shutter Island*](https://www.youtube.com/watch?v=x9lGthLqe3E), Andrew makes a statement that prompts the audience to question whether or not he’s actually sane and pretending to be otherwise.
Though he greets his doctor by the name of “Chuck” — the fictional partner he’s created for his alter ego — he follows it up with a thought-provoking question about their surroundings that he’s been pondering: “Which would be worse? To live as a monster or to die as a good man?"
While the line ultimately leaves viewers to decide whether Andrew is lucid and choosing to “die as a good man” by being lobotomized instead of living as the monster he believes he is, Dennis Lehane, the author of the book the film is based in, has weighed in with his thoughts on what the line means.
According to Lehane, who ended the book with Andrew's brief lucidity quickly passing, the film’s line is merely a “momentary flash” of the character’s sanity.

"My feeling was no, he’s not so conscious he says, ‘Oh I’m going to decide to pretend to be Laeddis so they’ll finally give me a lobotomy.’ That would just be far more suicidal than I think this character is,” he told MTV News (via [IndieWire](https://www.indiewire.com/features/general/shutter-island-ending-lobotomy-anniversary-1202212647/)).
Lehane continued, “I think that in one moment, for a half a second sitting there in that island he remembered who he was and then he asks that question and he quickly sort of lets it go. That was my feeling on that line.”
According to the film’s psychiatric adviser, Professor James Gilligan, however, who spoke to [*The Guardian*](https://www.theguardian.com/film/filmblog/2010/jul/29/shutter-island-ending) in 2010, Andrew was aware of his actions when he was ordered to be lobotomized.
The doctor revealed that he interpreted the line to mean, “I feel too guilty to go on living. I'm not going to actually commit suicide, but I'm going to vicariously commit suicide by handing myself over to these people who're going to lobotomize me."
