People often think that heart attacks are something that mostly happen to older men, not women. But heart disease is the leading cause of death for women in the U.S., and only about half of women know this.
1
Thatâs why itâs important to understand heart attack symptoms in women, which can be different from men.
Chest pain is common for both, but women often have other symptoms too.
2
These can include pain in the jaw, neck, or back, nausea, and shortness of breath.
3Â
It can make it hard for women to tell if theyâre having a heart attack.
Women are also more likely to think their heart attack symptoms are caused by anxiety.
4
This misunderstanding â combined with a wider range of symptoms â can cause women to wait longer to get treated.
âSeveral studies have shown that women wait longer to get treatment for a heart attack than men,â says Mingsum Lee, MD, a clinical cardiologist at Kaiser Permanenteâs Los Angeles Medical Center.
Once you know what to watch for, itâs easier to spot the signs and know when to seek care.
Heart attack warning signs in women
Recognizing the signs of a heart attack can be life-saving, especially since symptoms in women can be subtle or easily mistaken for other conditions.
Chest pain
The most common heart attack symptom for women (and men) is chest pain. âAbout 90% of women and men have chest pain when theyâre having a heart attack,â says Dr. Lee.
This chest pain and discomfort usually happens after stress â the stress can either be physical, like exercise, or emotional. The pain is usually strong, comes on gradually, and increases in intensity over several minutes.
âTypically, the pain feels very deep and itâs hard to localize or pinpoint,â says Dr. Lee. âPeople generally use terms like âpressure,â âsqueezing,â âheaviness,â or âtightnessâ to describe the sensation in their chest.â
Arm, back, neck, or jaw pain
A heart attack doesnât always cause pain just in the chest. It can shoot through your arm, neck, back â or even cause pain in the jaw. Sometimes it builds slowly, getting more intense over time.
Since most people expect pain to be in their chest during a heart attack, these other symptoms can be confusing. This is especially true because it may be difficult to pinpoint where the pain started.
Stomach pain
Nausea and stomach pain may also be heart attack signs for women. âSometimes people come in late for care because they think theyâre having
heartburn
or acid reflux,â says Dr. Lee. Heartburn or reflux comes from inflammation in the esophagus â the tube that leads from the throat to the stomach â which is right next to the heart. This can make it hard to tell if itâs discomfort from eating certain foods or a heart attack. âGenerally speaking, heartburn can be triggered by certain spicy food, citrus, and alcohol,â she explains. And acid reflux feels worse when you lie down.
Shortness of breath
You might be having a heart attack if you suddenly have shortness of breath for no apparent reason. It may feel like you have to stop and catch your breath while doing an everyday task. âFor example, if you can normally do grocery shopping with no problem, but suddenly you canât catch your breath while youâre walking down the grocery aisle and you have to stop to rest, thatâs a warning sign,â says Dr. Lee.
Sweating
Sudden sweating plus chest pain is another related heart attack symptom for women. You may break out in a cold sweat or feel clammy while also feeling some chest pain.
Fatigue
Similarly, chest pain with sudden fatigue may be a sign that youâre having a heart attack. You may feel overly tired for no reason â and the fatigue comes out of nowhere. Your regular activities suddenly become too difficult because youâre extremely tired.
How do you know if youâre having a heart attack?
Trust your gut. If something feels wrong, listen to your bodyâs warning signs and get checked out. Pay attention to new or unexpected symptoms. Donât wait for the classic crushing chest pain â every heart attack looks a little different.Â
Donât hesitate to call 911
You might not have all of these heart attack warning signs. But if youâre having any of them, call
911
immediately. Donât wait. Describe your symptoms and let the 911 operator know you may be having a heart attack.Â
In her work, Dr. Lee has seen both younger and older women put off going to the doctor â even when theyâre feeling heart attack symptoms. âYoung women are often focused on being the caretaker for their children or elderly parents, and they donât come into the hospital because thereâs no one else to take care of their children or parents,â she says.
On the flip side, Dr. Lee has seen older women who are widowed and live alone not want to bother their children or friends. âThese women may be having chest pain, but they donât want to bother people. So they sit at home and hope the symptoms go away,â she says. Sometimes, they donât drive and are too embarrassed to ask for help.
âI think a lot of times women are used to being the caregivers, so when they themselves need help they arenât used to asking for it,â Dr. Lee says. This could be another reason why women wait so long to get care for heart attacks.
But itâs important to listen to your body and prioritize your health.
Bottom line:
If youâre not sure if youâre having a heart attack, go to the hospital to get checked out. âThe earlier you come in for medical care,â Dr. Lee says, âthe earlier we can start therapy and the less damage there will be to the heart.â
Risk factors for heart attack in women
In addition to knowing key heart attack symptoms, itâs also important to know if you have risk factors for heart disease. âMany women arenât aware that theyâre at risk for heart attack,â explains Dr. Lee. âSo when they start having symptoms, they donât even consider that itâs a warning sign.â
Common risk factors for women include:
Certain medical conditions.
Women are at higher risk for heart disease if they have diabetes, high blood pressure, high cholesterol, or an inflammatory disease like rheumatoid arthritis or lupus.Â
Pregnancy complications.Â
Women who had pregnancy complications like gestational diabetes, gestational hypertension, or preeclampsia are at higher risk for a heart attack later in life.
Smoking.
Research shows that smoking can increase the risk of heart attack for young people. And women who smoke are 25% more likely to have heart disease than men who smoke.
5
Lifestyle choices.
Poor diet, overuse of alcohol, and physical inactivity all increase a womanâs risk for heart attack.Â
Menopause.
Lower levels of estrogen after menopause can increase the risk of heart attack for women.

# Heart attack symptoms in women: 6 warning signs
by Kaiser Permanente \|
October 20, 2025

People often think that heart attacks are something that mostly happen to older men, not women. But heart disease is the leading cause of death for women in the U.S., and only about half of women know this.1
Thatâs why itâs important to understand heart attack symptoms in women, which can be different from men.
Chest pain is common for both, but women often have other symptoms too.2 These can include pain in the jaw, neck, or back, nausea, and shortness of breath.3 It can make it hard for women to tell if theyâre having a heart attack.
Women are also more likely to think their heart attack symptoms are caused by anxiety.4 This misunderstanding â combined with a wider range of symptoms â can cause women to wait longer to get treated.
âSeveral studies have shown that women wait longer to get treatment for a heart attack than men,â says Mingsum Lee, MD, a clinical cardiologist at Kaiser Permanenteâs Los Angeles Medical Center.
Once you know what to watch for, itâs easier to spot the signs and know when to seek care.
## Heart attack warning signs in women
Recognizing the signs of a heart attack can be life-saving, especially since symptoms in women can be subtle or easily mistaken for other conditions.
### Chest pain
### Arm, back, neck, or jaw pain
A heart attack doesnât always cause pain just in the chest. It can shoot through your arm, neck, back â or even cause pain in the jaw. Sometimes it builds slowly, getting more intense over time.
Since most people expect pain to be in their chest during a heart attack, these other symptoms can be confusing. This is especially true because it may be difficult to pinpoint where the pain started.
### Stomach pain
Nausea and stomach pain may also be heart attack signs for women. âSometimes people come in late for care because they think theyâre having [heartburn](https://healthy.kaiserpermanente.org/georgia/health-wellness/healtharticle.heartburn-or-heart-attack-how-to-tell-the-difference) or acid reflux,â says Dr.
### Shortness of breath
You might be having a heart attack if you suddenly have shortness of breath for no apparent reason. It may feel like you have to stop and catch your breath while doing an everyday task. âFor example, if you can normally do grocery shopping with no problem, but suddenly you canât catch your breath while youâre walking down the grocery aisle and you have to stop to rest, thatâs a warning sign,â says Dr. Lee.
### Sweating
Sudden sweating plus chest pain is another related heart attack symptom for women. You may break out in a cold sweat or feel clammy while also feeling some chest pain.
### Fatigue
Similarly, chest pain with sudden fatigue may be a sign that youâre having a heart attack. You may feel overly tired for no reason â and the fatigue comes out of nowhere. Your regular activities suddenly become too difficult because youâre extremely tired.
## How do you know if youâre having a heart attack?
Trust your gut. If something feels wrong, listen to your bodyâs warning signs and get checked out. Pay attention to new or unexpected symptoms. Donât wait for the classic crushing chest pain â every heart attack looks a little different.
## Donât hesitate to call 911
You might not have all of these heart attack warning signs. But if youâre having any of them, call **911** immediately. Donât wait. Describe your symptoms and let the 911 operator know you may be having a heart attack.
**Bottom line:** If youâre not sure if youâre having a heart attack, go to the hospital to get checked out. âThe earlier you come in for medical care,â Dr. Lee says, âthe earlier we can start therapy and the less damage there will be to the heart.â
## Risk factors for heart attack in women
In addition to knowing key heart attack symptoms, itâs also important to know if you have risk factors for heart disease. âMany women arenât aware that theyâre at risk for heart attack,â explains Dr. Lee. âSo when they start having symptoms, they donât even consider that itâs a warning sign.â
Common risk factors for women include:
- **Certain medical conditions.** Women are at higher risk for heart disease if they have diabetes, high blood pressure, high cholesterol, or an inflammatory disease like rheumatoid arthritis or lupus.
- **Pregnancy complications.** Women who had pregnancy complications like gestational diabetes, gestational hypertension, or preeclampsia are at higher risk for a heart attack later in life.
- **Smoking.** Research shows that smoking can increase the risk of heart attack for young people. And women who smoke are 25% more likely to have heart disease than men who smoke.5
- **Lifestyle choices.** Poor diet, overuse of alcohol, and physical inactivity all increase a womanâs risk for heart attack.
- **Menopause.** Lower levels of estrogen after menopause can increase the risk of heart attack for women.

âI think a lot of times women are used to being the caregivers, so when they themselves need help they arenât used to asking for it,â Dr. Lee says. This could be another reason why women wait so long to get care for heart attacks.
But itâs important to listen to your body and prioritize your health.
