Complaint, Comment, and Compliment Form Use the U.S. Department of Transportation’s Office of Aviation Consumer Protection (OACP) Form below to file a complaint, comment, or compliment concerning your air travel experience when using or attempting to use an airline’s or travel agency’s services. Complaints should be filed with OACP when an airline or travel agency is not able to resolve an air travel problem to your satisfaction. OACP encourages you to first contact the airline or travel agency for help with an air travel problem as these entities may be able to take care of the problem on the spot. When you file a complaint, comment, or compliment with OACP, OACP will ensure that the appropriate airline or travel agency receives it. OACP will also direct that entity to respond directly to you with a copy to OACP when a complaint is filed. You can find more detailed information about OACP’s complaint handling process under the tab titled "Complaint Handling Process" Before you begin the process of filing a complaint, please have the following items ready: Booking details (ticket/itinerary/invoice), Flight details (dates/flight numbers/city pairs), and Any supporting documentation such as a copy of the complaint you filed with the airline or ticket agent if available.
